Non English original documents MUST be translated in English. Both original and English translated document MUST be submitted.

For updating of child's vaccination records
Copy of your child's birth certificate with both parents name
Copy of your child's Singapore Citizenship certificate / Dependant Pass / Long Term Visit Pass, Student Pass, Re-entry Permit, etc (If child is not born in Singapore)
Copy of both parent's Singapore NRIC/FIN ID documents (front & back image)
Copy of original vaccinations records
For updating of your own vaccination records
