Important Notes

  • All non-English documents MUST be translated to English. Both original and translated documents MUST be uploaded
  • Please ensure documents to be uploaded are clear
  • Maximum of 8 documents are allowed
  • Each documents size must be less than 2 MB
  • Only pdf,jpeg,jpg file format are accepted
  • Filename must be in English and do not contain special characters (e.g.%^#*-,etc)
  • If your child does not have a valid NRIC/FIN and one of the parents is Singapore citizen/permanent resident, please submit the request to update new vaccination records after child's NRIC/FIN is issued.
  • If your child is not born in Singapore and does not have a valid NRIC/FIN and both parents are non Singapore citizen/permanent resident, please submit your application at www.nir.cda.gov.sg/fcine/
