All non-English documents MUST be translated to English. Both original and translated documents MUST be uploaded
Please ensure documents to be uploaded are clear
Maximum of 8 documents are allowed
Each documents size must be less than 2 MB
Only pdf,jpeg,jpg file format are accepted
Filename must be in English and do not contain special characters (e.g.%^#*-,etc)
A valid NRIC/FIN is required to register new vaccination records with National Immunisation Registry.
