Patient consent is required before accessing their NIR immunisation records. By proceeding, you confirm that consent has been obtained from -

(a) if the patient is not a minor and has capacity to provide consent for him or herself, the patient; or

(b) if the patient is a minor or has no capacity to provide consent for him or herself, the patient's parent, guardian, donor, or deputy (as the case may be).

Welcome to the NIR Doctor Portal!

The National Immunisation Registry maintains the immunisation records recommended under the National Childhood Immunisation Schedule (NCIS) for those born from 1996 and the National Adult Immunisation Schedule (NAIS) taken after November 2017 of all Singapore residents, as notified by healthcare providers. Singapore Medical Council registered doctors can log in to this portal to update the immunisation records of their clients.

Doctor Access

All SMC registered doctors have access to NIR portal. There is no need to apply for user account through NIR portal. For first time user, please refer to FAQ-NIR Access Q7 to complete the Corppass user account setup.

Non-Doctor Access

For nursing homes/ eldercare institutions where in-house doctors are not available, please use the attached FormSG to apply for new user account(s) for clinic staff. Please submit the completed form together with the required documents (refer to FAQ - NIR Access Q8) to nir_account_access@cda.gov.sg. Alternatively, please use the attached form and submit all required documents.
